Sculptra, Radiesse, Profhilo and Bellafill: what biostimulators actually do
These are not fillers. They do not add volume — they make your face build its own, slowly. A different tool for a different problem.

A biostimulator does something else entirely — it provokes a controlled response so that your own body lays down collagen over the following months. Nothing happens quickly, and that is the point.

Sculptra — for overall facial volume loss
Poly-L-lactic acid particles that dissolve over weeks while stimulating fibroblasts to produce collagen. Treats the whole mid-face rather than a single fold, which makes it the right tool when the complaint is "my face looks deflated" rather than "this line bothers me". Usually a series of two to three sessions, spaced six weeks apart, and the result is still improving six months after the last one.
Not for you if
You want to see something next week, or you need a specific, defined change in one small area. Sculptra is diffuse and slow by design.
Radiesse — structure now, collagen later
Calcium hydroxylapatite in a gel carrier. It provides immediate lift because the microspheres take up space, then continues to work as they stimulate collagen and the carrier resorbs. Strong along the jawline and for deep folds; not used in lips.
Hyper-diluted Radiesse
The same product diluted heavily and placed superficially. At this dilution it adds no volume at all — it is used purely for skin quality, tightening crepey texture on the neck, décolleté, upper arms and knees. One of the few genuinely effective options for skin that is thin rather than lax.
Profhilo — hydration, not volume
Very high concentration, ultra-pure hyaluronic acid with no cross-linking, so it spreads through the tissue instead of holding a shape. It hydrates from within and stimulates a mild collagen and elastin response. The standard protocol is two sessions a month apart. It will not lift anything — its job is skin that reflects light properly again.
Bellafill — the five-year one
PMMA microspheres suspended in bovine collagen. The collagen gives immediate correction and resorbs; the microspheres remain permanently and act as a scaffold for your own collagen. FDA-approved for nasolabial folds and acne scarring, and it lasts around five years.

- Skin is crepey rather than loose → hyper-diluted Radiesse
- Skin looks dull and dehydrated → Profhilo
- Established patient wanting long-term correction of a specific fold or scar → Bellafill
Biostimulators and fillers are not competitors. A common plan uses a biostimulator for the foundation and a small amount of HA filler for definition on top — built in that order, not the reverse.
Common questions
Why is Sculptra more expensive if I see nothing for two months?
You are paying for a result that lasts two years or more and treats the whole mid-face. Per month of effect it is one of the better-value treatments; it just front-loads the cost and back-loads the result.
Can biostimulators be dissolved?
Sculptra and Radiesse cannot be dissolved but they do resorb over time. Profhilo is hyaluronic acid and can be dissolved. Bellafill is permanent and cannot be removed — which is exactly why it is used sparingly.
Can I have a biostimulator and filler together?
Yes, and it is common. The usual sequence is the biostimulator first to rebuild the foundation, then a conservative amount of filler for definition once the collagen response has settled.
Are these safe?
All four are FDA-approved and have long safety records. The main risk with the collagen stimulators is nodule formation, which is minimised by correct dilution, correct depth and post-treatment massage where the protocol calls for it.
